Internal Grant Update: Hospital to Home

Hospital to Home is exceeding expectations. They are making a true impact on patient’s lives.

The Foundation and Medical Social Services partnered with The Duke Endowment to provide patients with extra care.

Here’s what patients have to say about Hospital to Home:

“I am a prime example of what is possible when given the opportunity and resources to go home. Without this help I would have had to be placed in a facility, for that I am very thankful to LMC and the grant program.”

- Hospital to Home Patient

“It had a tremendous impact on my life; I was stuck in a rut due to my condition. I was not even able to unpack my things into my apartment and I had moved in almost 6 months ago, the staff was friendly and helped me with everything from setting up my meds to helping get to appointments and even finally unpack my belongings. It was a great benefit for me.”
