Diabetic patients got a big gift recently. The Foundation has awarded a grant to provide diabetic supplies to Lexington Medical Associates (LMA) patients and patients at LMC Batesburg and LMC Swansea.
Many patients at LMA, Batesburg, and Swansea who have diabetes are unable to purchase the glucose meters and testing strips to check their blood sugar levels, due to the high costs.
Through this grant, social workers identify patients in need and provide each with a Reli-On meter and box of Reli-On strips to help create compliance and continuity of care from the inpatient to outpatient setting.
